Healthy synovial fluid is rich in hyaluronic acid, a molecule that gives it the viscosity to lubricate cartilage and absorb shock. In osteoarthritis, the concentration of hyaluronic acid in the joint drops. Viscosupplementation replaces some of it through direct intra-articular injection. The mechanism is not just mechanical — hyaluronic acid also has anti-inflammatory and chondroprotective signaling effects.
Single injection or a short series, under ultrasound guidance.
Different commercial preparations require one injection, three injections, or five injections at weekly intervals. Each injection is performed under ultrasound guidance to confirm intra-articular placement. The injection itself takes a few seconds. Most patients tolerate it well.
For knee osteoarthritis that has not responded to conservative care.
Viscosupplementation is FDA-approved for knee osteoarthritis. The best candidates are patients with mild to moderate knee OA who have not gotten adequate relief from activity modification, weight optimization, NSAIDs, and physical therapy, and who are not yet ready for joint replacement. Evidence is weaker for other joints; coverage varies.
Effect builds over weeks. Often lasts six months or longer.
Expect mild soreness for one or two days after each injection. The benefit builds over two to four weeks and frequently lasts six months or longer. Patients can repeat the course every six months as needed. Pairing viscosupplementation with a focused strengthening program produces better outcomes than either alone.
