What It Is

The joint that bears the load between your spine and your legs.

The sacroiliac (SI) joint connects the base of your spine to your pelvis. It is a small, strong, low-motion joint that carries enormous load. SI dysfunction is one of the most under-recognized causes of low-back pain — many patients are treated for lumbar disc disease for years before the SI is identified. Tenderness over the SI joint, pain with single-leg stance, pain centered below the belt line and into the buttock, and pain after pregnancy or trauma are classic clues.

How It Works

Fluoroscopic guidance into a joint that is easy to miss.

You lie prone. Under live fluoroscopy, a needle is advanced into the inferior aspect of the SI joint. Contrast confirms intra-articular position before any medication is delivered. Anesthetic and a small dose of corticosteroid are injected. The procedure takes ten to twenty minutes. No general anesthesia is required.

Who It's For

Indicated for confirmed or suspected SI dysfunction.

SI joint injection is appropriate for unilateral low-back or buttock pain with positive provocative SI tests on exam, sacroiliitis on imaging, post-partum or post-traumatic onset, or pain that has not responded to lumbar-directed treatments. We also use it diagnostically before considering SI joint fusion.

The anesthetic gives a several-hour window of relief that is itself diagnostic — strong, immediate response tells us the SI joint is the generator. The corticosteroid builds over three to seven days. Many patients get months of relief from a single injection. For those whose pain returns, we may repeat, move to SI radiofrequency ablation, or evaluate for SI joint fusion.
