What It Is

A cell-rich concentrate harvested from your pelvis.

BMAC is prepared by aspirating a small volume of marrow from the iliac crest — the back of your pelvic bone — and concentrating it to produce a high density of mesenchymal stromal cells, growth factors, and other regenerative components. That concentrate is injected into a joint, tendon, or other target tissue under ultrasound or fluoroscopic guidance.

How It Works

A two-step procedure performed in one visit.

You lie prone under local anesthetic with optional light sedation. A trocar is advanced through numbed skin into the back of the iliac crest, and bone marrow is aspirated into a sterile collection system. The aspirate is processed in a centrifuge to concentrate the cellular fraction. The concentrate is then injected — under imaging guidance — into the target joint or tendon. Total appointment time is about ninety minutes.

Who It's For

For moderate arthritis and select tendon injuries where PRP has plateaued.

BMAC is reasonable for moderate knee, hip, or shoulder osteoarthritis where the patient is seeking to delay joint replacement, for select rotator cuff and gluteal tendinopathies that have not fully responded to PRP, and for select bone-stimulation indications in delayed healing. Evidence is evolving; we are transparent with every patient about what the published data does and does not support.

Recovery & Results

Soreness for several days. Effect builds over months.

Expect soreness at both the aspiration site and the injection site for a few days. Walking is encouraged immediately. We restrict high-impact activity for two to four weeks. The biologic effect builds over three to six months. A single procedure is the most common protocol, with reassessment at three months to decide on further intervention.
