Burning, tingling, electric, or numb pain in a glove-and-stocking distribution. Often worse at night. Loss of fine sensation. Sometimes balance changes and gait instability.
Your first visit.
Focused neurologic examination. EMG/NCS to characterize the neuropathy. Review of contributing factors. Coordination with primary care for systemic workup when indicated.
Pharmacology, neuromodulation, and supportive care.
First-line pharmacotherapy as appropriate. Topical agents where applicable. For refractory cases, spinal cord stimulation has growing evidence in select neuropathies. Peripheral nerve stimulation for select isolated nerve distributions. Adjunctive PT for balance, strength, and fall prevention.
