Sharp, electric, or burning pain starting at the base of the skull and radiating up over one side of the head. Scalp tenderness over the affected area. Sometimes triggered by neck position or contact with the back of the head.
Your first visit.
Examination identifying tenderness at the occipital notch and characteristic referred pain pattern. A discussion of trigger factors and prior treatments. A trial of occipital nerve block — typically diagnostic and therapeutic in the same visit.
Block, then plan from there.
Occipital nerve block is both diagnostic and therapeutic. Patients with strong response often benefit from repeat blocks every several months. For those with strong but short-lived response, radiofrequency ablation of the occipital nerve or referral for occipital nerve stimulation are options.
